We thank Dr. Taylor for her interest in and discussion of our article!1 I wholeheartedly agree that many of the issues highlighted in this article about neurodegenerative diseases also apply to a neuro-oncology patient population. Issues of "when to begin 'big picture' discussions", surrogate decision-making as a patient's cognitive and communication impairment progress, how to approach acute medical setbacks within the larger context of disease progression or neurocognitive decline are all highly relevant to the neuro-oncology world. Thanks for bringing this to light.
